      Care Navigator


      Looking for a way to make a difference in people’s lives?

      As a Care Navigator, you will play a critical role in supporting our members to live the best, rest of their lives. This position acts as the primary point of contact for our members, their families, and our partner senior living facilities. You will be able to help members navigate through the ever-changing healthcare landscape and act as an advocate to ensure they receive the highest quality of care.

      Position is remote and requires visits to facilities on a regular basis. Duties may include, but are not limited to the following:

      Member Engagement:

      • Main point of contact for Members and families upon Plan enrollment
      • Preform welcome interview and Health Risk Assessment, routine follow up with members/families
      • Assist members with items including, but not limited to referral management, appointment scheduling, DME ordering, and accessing Plan benefits
      • Participate in Care Planning meetings as appropriate
      • Coordinate care between specialists, home health, therapy, Plan APP, PCP, etc.

      Facility Engagement:

      • Act as the point person for facilities, connecting them to Plan resources such as APP, sales, leadership
      • Liaison between facilities and the Plan to assist with customer service requests such as billing questions, authorization needs, or member transfers

      Clinical Support:

      • Medical records collection: obtain clinical records for current and new members from community sources (H&P’s, medication history, specialist notes, etc.)
      • Other record collection as required to support Utilization Management and APPs
      • Monitor for any transitions of care and notify the Plan APP and utilization management
      • Monitor for any changes in enrollment status and notify Plan APP

      AgeRight Advantage is a provider owned Medicare Advantage Plan that specializes in providing care and services to members residing in Senior Living Facilities.       Qualifications

      • At least 1 year experience working in health care, social services, or other direct patient engagement experience
      • Experience in senior living facilities is strongly preferred
      • Computer literate, able to use email, the internet and web searches as standard applications and databases.
      • Excellent communication skills, both verbal and written; able to supply reports as required.
      • Pleasant personality and team player
