      Job summary

      We have a vacancy for a Care Navigator to work in our busy GP Practice. If you would like to join a small friendly team this is the surgery for you. We have a well established team and we look forward to welcoming you into our team

      Main duties of the job

      Act as chaperone in accordance with Practice policy Clear rooms after surgeries Ensure building security have thorough knowledge of doors/windows/alarm. Registrations of new patients computer data entry and medical records. Process patients change of address computer data and medical records (have knowledge of practice area. Process repeat prescription request in accordance with practice guidelines. Receiving patients consulting with members of practice team Handing completed repeat prescriptions to patient and checking names and address. Be able to cover all reception position as necessary Any other tasks allocated by managers

      We are a GP practice in Stafford, we have a list size of just under 10300 patients. We are a busy practice with 6 GP's, 2 ANP's, HCA's and 2 Practice Nurses. We are a friendly team and we strive to give the best care we can

Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone Job responsibilities: Administration To have a thorough knowledge of all practice procedures. To work in accordance of written protocols Pulling/filing notes for surgeries and update as necessary Filing post in medical records Fax and photocopy as requested Reception Receiving patients consulting with members of practice team Handing completed repeat prescriptions to patient and checking names and address. Be able to cover all reception position as necessary Appointments Process appointment requests for today future appointments from patients by telephone and in person. Deal with visits requests Computer Registrations of new patients computer data entry and medical records.
