Overview: This internship will offer an MHA student interested in healthcare business a comprehensive view of the inner workings and elements of a company that are required to come together to make a successful care delivery enterprise. The intern will have a chance to experience and observe our Operations, HR, Finance, Logistics, and Strategy management. Most of the day-to-day work will be in the form of projects. These projects will help to build an understanding of the company and health service delivery as a whole. The intern will participate first-hand in efforts to effectively manage patients after they’ve been discharged, which is a key feature distinguishing high-performing health systems from their peers. The Continuum Integration Center operationalizes new models of care that take discharged patients’ comprehensive needs into consideration, which effectively will help shift delivery of care toward a patient-centered, health management approach that spans the continuum to achieve the Triple Aim.
A key mission of the Senior Services | Continuum Integration | VCU Health at Home team is intentional focus on person-centered, holistic care, which is essentially putting emphasis on the patients’ clinical needs in the context of the social dynamics inherent within their everyday lives. To understand the impact of all of the aspects that affect a patient’s health journey, a solid model of care coordination is needed to anticipate health needs and work towards a preventive approach to medicine. Implementing a patient-centered model means putting the patient at the center of their own health care journey, supporting and providing them with the tools and resources necessary to thrive across a variety of care settings. We provide and promote care coordination through the collaboration and alignment of various stakeholders such as our hospitals, clinics, patients’ health plans and integrated community partners like SNF, Home Health, Hospice, and many more.
The intern will observe and participate in the VCU Health organization’s strategic investments in areas such as:
• Analytics and data management
• Population health programs like CMS Bundled Payments and ACO Reach
• Virtual Care including Remote Patient Monitoring
• Ambulatory Care
• Community Care: Nursing Home, Home Based Primary Care, Home Hospital
• Post–hospital discharge continuity of care programs and nurse case management
• Marketing and Educational resources
• IT infrastructure and EMR
• Process Improvement
• Legal and contracting Licensure, Certification, or Registration Requirements for Hire: None Licensure, Certification, or Registration Requirements for continued employment: None Experience REQUIRED: Previous experience using a personal computer and various software applications, including Microsoft, e-mail, etc.; Strong communication and interpersonal skills Experience PREFERRED Previous work experience in a healthcare setting Education/training REQUIRED: High School diploma or equivalent Education/training PREFERRED: Post High School courses in an allied health or healthcare related field from an accredited program Independent action(s) required: None Supervisory responsibilities (if applicable): None Additional position requirements: Must be prepared to work rotating shifts, weekends, holidays, extended hours and during inclement weather to ensure patient flow and throughput Age Specific groups served: N/A Physical Requirements (includes use of assistance devices as appropriate): Physical: Lifting Less than 20 lbs. Director, Continuum Integration Operations. Also responsible to each manager that the intern is currently completing projects for. All managers will be located at the Continuum Integration Center, in Richmond VA.
Work Hours:
15-20 hours/week during semesters. To be arranged based on the intern’s schedule, within the normal VCU business hours: 8:00am-5:00pm Monday – Friday.During the summer full-time would be an option based on the desires of the intern and the company. When the intern is in their semester a schedule will be created based around their classes and needs.
