What to Expect (Job Responsibilities):
- Identify and coordinate internal and external resources for patients and families post-hospital discharge
- Create care plans for high-risk patients to reduce readmission rates
- Maintain caseload of patients to ensure proper follow-up care
- Facilitate communication between various healthcare disciplines for coordinated patient care
- Collaborate with medical staff and ancillary services to expedite care delivery
What is Required (Qualifications):
- Registered Nurse license issued by the state
- Bachelor's Degree in Nursing preferred
- 2 years of experience in nursing in acute care or community setting
- Excellent written and verbal communication skills
- Strong organizational and problem-solving skills
