GENERAL SUMMARYThis position coordinates all care pertaining to occupational health patients. They serve as the source of communication for payers, employer, providers and the patients care team. The case coordinator acts as a company and patient advocate. This person is responsible for scheduling patient appointments, maintaining appropriate documentation, acquiring insurance authorization for ongoing treatment, tracking the patient's participation and communicating patients' progress to the appropriate individuals. Serves as a resource to management on business line opportunities, service utilization, business decision outcomes and cost effectiveness of services. The case manager will assist with marketing occupational health to companies in the community. In conjunction with the manager, functions in a leadership and educator role for the practice to ensure compliance with occupational health standards and work flows. This position requires the ability to work effectively with providers and ancillary staff to affect quality patient care. Job duties include providing professional nursing care including but not limited to; treatments, patient assessments, assisting with medical procedures, respirator fit testing, drug screening, breath alcohol testing, audiometry and pulmonary function testing.ESSENTIAL DUTIES AND RESPONSIBILITIES(40%)* Completes required documentation and communicates pertinent information in a timely manner to the employer and other members of the patient’s care team. This includes, although not limited to: all results of pre-employment screening, drug testing, alcohol testing, Pulmonary Function Testing (PFT), audiograms, work injury status and other testing occupational health specific to certain urgent care business lines.(30%)* Provides case management for all work-related injuries/illnesses for occupational health patients in collaboration with the health care providers. Completes thorough documentation in a professional and timely. Obtains and assures ongoing insurance authorization for treatment. Documents and communicates insurance limitations or barriers to the provider for consideration in managing the patient's care plan(20%)* Performs occupational health/urgent care duties as defined by the practice. This may include, although not limited to, assistance with leadership of the practice, conducting patient physical assessments, drug and alcohol testing, immunization administration, PFT’s, respirator fit testing, and hearing testing.(10%)* Educates staff and providers on protocols, work flows and regulations related to occupational health such as; Occupational Safety and Health Administration (OSHA), Code of Federal Regulations (49 CFR Part 40) specific to drug and alcohol testing, Department of Transportation (DOT) physical examination requirements and all other related occupational health services. Requires ability to stay current on regulations, and communicate this information in a clear, concise and professional manner to other members of the urgent care team.OTHER DUTIES AND RESPONSIBILITIESDevelops professional business relationships with referral sources, service providers, business/industries, insurance and worker’s compensation representatives, and external case managers.Trains and certifies all staff for drug and alcohol testing.Participates in marketing of occupational health services through community promotions, meetings and presentations.Performs referral development activates to enhance the visibility of the occupational health services marketing, ultimately leading to an increase in volumes and revenues.Interfaces with the service providers to communicate timely, pertinent information which impact's delivery of care or reimbursement to enhance cost-effective outcomes.Obtains prior authorizations for patient care follow up orders, communicates any challenges of treatment/testing approval to providers and employers.Coordinates modifications to patient treatment plan.Responsible for coordinating, scheduling and providing off-site occupational health services at companies in the community.Responsible for completing drug and alcohol testing for post-accident and for cause requests per practice protocol.Based on practice needs and protocols, performs respirator fit testing according to established procedures.This includes knowledge of various mask types and proper mask fit according to OSHA regulations.Performs post-exposure counseling and monitoring for follow up care needed for Human Immunodeficiency Virus (HIV) and communicable diseases.Performs required care/occupational health/employee health services to MyMichigan Health employees for annual wellness screenings, work related injuries or for cause drug and alcohol testing.Based on practice needs, may assist clinical staff in urgent care.Prepares statistical reports as requested by manger.Provides support and input on outcomes of annual company satisfaction survey.Communicates in a clear, effective and professional manner.Is able to be easily adapt to changing work environments, and clientele.Demonstrates excellent leadership, customer service, organizational and time management skills.MyMichigan Health is a technology driven organization and employees need to demonstrate competency in Microsoft® Windows. An employee may be required to participate in further learning opportunities offered by MyMichigan Health.CERTIFICATIONS AND LICENSURESBLS - Basic Life SupportPreferred Equivalent Experience - BLS certification is required within 90 days of hire or transfer date.RN - Registered NurseRequiredREQUIRED EDUCATIONEducation: Associates Degree Comments: Bachelor's degree is nursing is preferred.OTHEREXPERIENCE, TRAINING AND SKILLS:One to three years’ experience related nursing experience.Medical Review Officer – Assistant within 3 months of employmentCouncil for Accreditation in Occupational Hearing Conservation (CAOHC) certified within six months of hire, as needed per practice.Professional Drug Collector & Trainer certification within 6 months of hire.Certified Breath Alcohol Trainer preferred or acquired within 6 months of hire.Must have a valid driver’s license or adequate transportation to local occupational health businesses or meetings.Must possess the skills to lead and educate patients, colleagues and community members..The ability to develop professional/business relationships with companies.Experience in urgent care/occupational health preferred.Certified Occupational Health Nurse preferred.Experience with insurance reimbursement/authorization and worker's compensation.Experience with sales or business networking, business planning, and data analytics and/or electronic data systems preferred.PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONSExposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.Overall vision and hearing is necessary with or without assisted device(s).Frequently required to sit/stand/walk for long periods of time.May require frequent postural changes such as stooping, kneeling or crouching.Some exposure to blood borne pathogens and other potentially infectious material.Must follow MyMichigan Health bloodborne pathogen and TB testing as required.Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.Overall dexterity is required including handling, reaching, grasping, fingering and feeling.May require repetition of these movements on a regular to frequent basis.Physical Demand Level: Light. Must be able to occasionally (0-33% of the workday) lift or carry 11-20 lbs., frequently (34-66% of the workday) 10 lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls.About the company
