Schedule: Full Time
Shift: Days, Every Sat/Sun and a day during the week along with holiday rotation. 8-430pm

The Case Management Assistant supports the Case Management staff by providing clerical and technical support for the department's professional associates to facilitate and execute discharge plans and other related activities. The CMA may provide administrative support for the Care Management Department in the absence of department administrative assistant.
The value you’ll bring to the team:
· Provides quality customer service to external and internal customers both on telephone and in person to ensure courteous and professional experience.
· Provides clerical support to department to ensure department efficiency and staff satisfaction.
· Provides Medicare Important Message (IMM), MOON and other compliance forms as requested by Care Managers, per policy
· Assists with sending of referrals and auth requests for services/ placement through NaviHealth
· Assists with final booking of Navihealth referralsThe expertise and experiences you’ll need to succeed:
· High School Diploma or equivalent
· At least one-year experience in acute care settingAbout the company
