      What to Expect (Job Responsibilities):
      - Maintain internal data and reporting for the Case Management team, ensuring compliance with payer requirements
      - Validate monthly member reports to identify potential members for various management services
      - Assist Nurse Case Managers in preparing reports and compiling discharge lists for follow-up
      - Conduct outreach calls to evaluate member needs and complete Health Risk Assessments
      - Provide clerical support, including document preparation and obtaining medical records

      What is Required (Qualifications):
      - Associate's Degree in Health Care, Business, or a related field preferred
      - Minimum of three years of experience in Managed Care, Case Management, or a Medical Office environment
      - Experience in a multi-tasking environment is required
      - Typing speed of 30-35 words per minute recommended
      - Strong organizational and communication skills

      How to Stand Out (Preferred Qualifications):
      - Experience with data tracking and analysis in a healthcare setting
      - Familiarity with case management software and tools
      - Previous outreach experience in a healthcare context
      - Ability to work independently and manage multiple priorities
      - Knowledge of health plan compliance and CMS requirements
