      POSITION SUMMARY

      Under the supervision of the Senior Program Director, the Case Manager and Outreach Coordinator play a vital role in delivering high-impact transitional housing and community health outreach services. This position provides case management for up to 12 families enrolled in a 24-month transitional housing program, helping them achieve sustainable, long-term housing and personal stability. In addition to direct casework, the position will support the broader Community Health Team, engaging in outreach efforts that connect individuals and families to health-related resources and wraparound services.

      The Case Manager and Outreach Coordinator will coordinate life skills training, conduct assessments, provide referrals, and ensure compliance with funding and program guidelines all while promoting the organization's overall mission and values.

      ESSENTIAL RESPONSIBILITIES

      Case Management (Transitional Housing)

      • Manage a caseload of up to 12 families, supporting them throughout their transitional housing journey.

      • Conduct comprehensive assessments to identify client goals, barriers, and resource needs.

      • Collaborate with each client to create and implement an Individual Service Plan (ISP) with clearly defined goals and timelines.

      • Deliver strengths-based, trauma-informed case management, including crisis intervention, counseling, and referrals to internal and external supports.

      • Monitor client progress and adjust service plans as needed to ensure success in achieving permanent housing and life stability.

      • Facilitate workshops and individual sessions on life skills development (e.g., budgeting, job readiness, parenting, and wellness).

      Program Administration and Compliance

      • Maintain accurate, timely, and complete case documentation in compliance with internal standards and funder requirements, including HMIS.

      • Support the preparation of reports, grant deliverables, and site visits from funders or internal monitors.

      • Conduct periodic file audits to ensure quality assurance and compliance.

      • Assist in developing and maintaining systems to track service outcomes and impact.

      Community Health Outreach

      • Serve as a liaison between the transitional housing program and the Community Health Team.

      • Identify outreach opportunities to engage community members in health-related services such as preventative care, SNAP enrollment, and mental health supports.

      • Support the coordination of community events, resource fairs, and group presentations related to health education and social services.

      • Build and maintain partnerships with community-based organizations and health service providers to strengthen referral networks.

      Team Collaboration and Development

      • Participate in regular team meetings, training, and continuous quality improvement initiatives.

      • Support a collaborative and mission-driven work culture rooted in the values of Caring, Honesty, Respect, and Responsibility.

      • Assist with other duties as needed to support program goals and team effectiveness.

      QUALIFICATIONS

      • Bachelor’s degree in Social Work, Human Services, Public Health, or related field required; Master’s degree preferred.

      • Minimum 2 years of experience in case management, transitional housing, or community outreach.

      • Strong understanding of trauma-informed care, housing-first principles, and social determinants of health.

      • Knowledge of HMIS or similar case management databases preferred.

      • Excellent communication, organizational, and documentation skills.

      • Ability to work independently and collaboratively in a fast-paced, client-centered environment.
