- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field.
- Previous experience in case management, social work, or related roles, preferably in behavioral health or crisis settings.
- Knowledge of public and private benefit programs and community resources related to Social Determinants of Health (SDOH).
- Excellent communication and interpersonal skills to engage effectively with individuals and colleagues.
- Empathy, compassion, and a person-centered approach.
- Ability to work collaboratively in a multidisciplinary team environment.
- Strong organizational and time management skills to manage case management responsibilities effectively.
- Familiarity with electronic health records (EHR) systems and healthcare technology.
- Bilingual is a plus.

Merakey is looking for a Case Manager to work weekends at our Behavioral Health Urgent Care Clinic in Philadelphia.
Sat and Sun 10:30 AM - 7:00 PM
Earn $28.40 per hour
The Case Manager provides immediate and follow-up case management services to individuals experiencing urgent behavioral health challenges. The primary responsibility will be to offer comprehensive support, including benefit enrollment, information about external resources, addressing the Social Determinants of Health, referrals, and completing outreach contact calls. The position will require a collaborative and empathetic approach to help individuals access the resources necessary to address their health needs and improve overall well-being.
