      Case Manager (Bilingual Preferred)

      EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 62,888 people in need through 100 programs across Long Island and NYC, seeks a Case Manager (Bilingual Preferred) to work full time for its Queens Treatment Alternatives for Safer Communities (TASC) Program.

      The Case Manager (Bilingual Preferred) position is anon-exempt position paying $23-25 per hour at 35 hours per week. The hours of operation are Monday-Friday 9:00AM-5:00PM and the office is located in Queens, NY. The role reports to the Program Director.

      Primary Purpose of Job:

      The Case Manager is responsible for court liaison, assessment, referral and monitoring of clients involved in the criminal justice system into appropriate treatment modalities, management of a caseload, conducting cognitive behavioral interventions and providing HIV/Hep C interventions and referrals to testing.

      Principal Duties & Responsibilities:

      1. Act as a liaison to the courts. Conduct assessment interviews and arrange for the client to be placed into the appropriate treatment program. Investigate and arrange client service plans.
      2. Complete follow-up paperwork and maintain complete client records and have them available for inspection by Supervision at any time. Prepare or obtain monthly and quarterly progress reports as required by funders.
      3. Prepare and present written status reports to the criminal justice system in a timely manner regarding client's progress.
      4. Investigate and arrange supportive services for clients.
      5. Escort clients to appointments and treatment facilities when indicated.
      6. Maintain a caseload of offenders.
      7. Make monthly site visits to all treatment agencies assigned to caseload.
      8. Conduct drug testing on clients of the Program as needed.
      9. Conduct cognitive behavioral therapy interventions; provide HIV/Hepatitis testing resources as well as awareness, prevention, and referrals to treatment.

      Knowledge, Skills, & Abilities Required:

      1. Associates degree in a helping profession and at least 2 years of prior experience working in intervention programs with criminal justice involved participants. Bachelors degree preferred.
      2. Experience conducting health education groups and or cognitive-behavioral interventions preferred.
      3. Knowledge of treatment programs and local criminal justice and health systems preferred.
      4. Good communication skills, both oral and written; Ability to work cooperatively with treatment and criminal justice systems.
      5. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.
      6. Bilingual (English/Spanish) BIG PLUS/HIGHLY PREFERRED!!!
