      Job Location: TCN Christopher House - Xenia, OH
      Salary Range: $18.00 - $19.00 Hourly

      Description

      Provide individual and group counseling under clinical supervision, supporting residents' growth and the achievement of treatment goals while maintaining a safe and clean environment. This role includes completing intakes, designing activities, monitoring health and medication, and ensuring adherence to house rules. The Case Manager also documents client progress, maintains open communication with stakeholders, handles crises, and participates in ongoing training and collaboration with other agencies.

      Primary Responsibilities:

      • Reports daily activities and other pertinent information.
      • Documents client progress and activities in residential charts and in daily shift reports.
      • Assists in the preparation of facility summary reports.
      • Engages in ongoing communication with community support personnel, supervisors, and other pertinent clinicians to relay information and updates.
      • Maintains 100% accuracy in documentation and summary reports.
      • Completes documentation according to TCN standards.
      • Provides only those services authorized by Utilization Management and provides services within the authorized frequency and duration of services.
      • Individualized Service Plan, Progress Notes and other documentation clearly identifies consumer’s input in services by having consumer’s signature when clinically appropriate.
      • Completes progress notes at the end of each session and submit all documentation within 24 hours of the service provided.
      • Develops and maintains quality, accurate clinical charts for all clients served.
      • Handles crisis situations, as needed, calmly and effectively.
      • Maintains agency-required annual training including client rights and grievances, safety training, AEGIS (de-escalation) training, infection control, and others as required by Ohio MHAS.

      Work Location and Standard Work Hours:

      • Xenia, OH
      • Full-time
      • Some evenings and weekends may be required

      Qualifications


      • Education: Associate degree in human/social service field.
      • Experience: One year in the field of alcohol and substance abuse residential treatment preferred.
      • Certification: CDCA or above required, SWA or above preferred; CPR; First Aid; AEGIS (De-escalation) training; Medication Monitoring, Food Service Manager certification.
      • Other: Valid Ohio driver’s license; proof of automobile insurance; meet ODADAS licensure and certification requirements; demonstrated competencies for working with persons with substance use and/or mental health disorders.
