      Position Title: Case Manager - Homeless Services

      Classification: Full-Time, Salaried, Exempt

      Schedule: Full time, Monday-Friday

      Location: Guild, 122 Wabasha St S. Suite 400 St Paul, MN 55107, USA

      Salary Range: $46,000 - $50,000 annually

      Purpose:

      This position is responsible to engage and provide Case Management to individuals experiencing homelessness who have been assessed within a Coordinated Entry system in the metro area. The Homeless Services Case Manager helps adults with serious and persistent mental illness gain access to medical, social, educational, vocational, financial, and other services that may be necessary to address mental and physical health needs. This position is a unique combination of providing mental health case management while supporting individuals who have experienced or are experiencing homelessness.

      Key Responsibilities:

      • Provide flexible, individualized support so individuals can lead self-directed lives.
      • Assess needs, identify goals, and plan services so that individuals’ needs are met, and goals are enhanced.
      • Communicate consistent information and aid the team to ensure continuity of service for the individuals served.
      • Access available community and housing resources so that necessary and relevant resources match individual’s needs and preferences.
      • Facilitate communication among others involved, to provide coordination and define service direction to attain individuals’ goals.
      • Provide crisis assessment and intervention so individuals receive the assistance they need to be safe in their home and community.
      • Ensure services are documented in individual records so information is readily available, results of services can be determined, and record-keeping standards are met.
      • Promote an inclusive, diverse, culturally competent and respectful workplace.

      Skills needed for success in this role:

      • Strong verbal communication and human relations skills including the understanding of boundaries, values, and personal differences.
      • Well-developed organizational and time management skills including the ability to meet deadlines and prioritize competing activities.
      • Skill in exercising judgment, knowing when to use own discretion and when to seek supervision and guidance.

      Ability to:

      • Problem solve, including the ability to independently identify the essential factors in situations and actively engage individuals in solutions.

      • Easily establish rapport with participants who live within a culture of poverty, have a mental illness, and/or a chemical health issue, and who may be homeless.
      • Develop and maintain receptive, open, and approachable lines of communications with clients, social service providers, community providers and county staff.
      • Establish and maintain positive working relationships with others.
      • Be flexible, think creatively and explore new alternatives.
      • Take a task start to finish and follow up with others to assure the task is complete.

      Education and Experiences:

        The Case Manager shall be qualified under Minnesota law as follows:

        • Holds a bachelor’s degree in one of the behavioral sciences or related fields from an accredited college or university and has 2,000 hours (1 year) of experience in the delivery of service to persons with mental illness

        • Meets the definition of mental health practitioner (https://www.revisor.mn.gov/statutes/cite/245.462)

        Additional requirements include:

        • A valid MN Class ‘D’ Driver’s License, reliable vehicle, satisfactory driving record and necessary insurance.
        • Ability to perform CPR, First Aid, and respond to emergency situations.
        • Ability to use electronic health records for service documentation and web-based housing information systems.
        • Must not have received services from Guild Incorporated within the last two years

        Working Conditions:

        The position is based at Guild St. Paul Area; however, most work is in the field as services are provided to individuals wherever needed. The position requires transporting individuals in personal vehicle regularly. This position has the potential to encounter bodily fluids, communicable diseases, and/or tobacco smoke.
