- Build meaningful and purposeful client relationships with appropriate boundaries.

- Complete 3 or more creative attempts per client if needed.
- Use creative strategies to engage in goal planning.
- Engage and build rapport with clients.
- Assess eligibility for program and services.
- Connect individuals to desired and necessary services.
- Develop Goal Plan.
- Bachelor's Degree in a Human Services related field.
- 2,000+ hours of supervised experience working with adults with mental illness.
- Ability to provide Rule 79 Adult Mental Health Targeted Case Management per DHS guidelines.
- One or more years of experience working with single adults experiencing homelessness or similar experience.
- Lived experience will be valued and considered as part of the experience requirement.

The Case Manager, Mental Health provides Rule 79 Case Management services to adults with mental illness, with an utter commitment to help them live successfully in their goal living situation. This position also assists adults with serious mental illness in pursuing the outcome of improved or maintained mental health functioning. This is accomplished by finding a safe, affordable placement; providing necessary support services; and assuring that any required additional services and providers are informed and in place.
