- Bachelor's degree in human services, Psychology, Social Work or a related field.
- Minimum of 2 year working in case management, mental health, community outreach, and/or providing services to the homeless or veteran populations.
- Experience working with military veterans in various care settings.
- Must have a valid driver’s license and be able to be insured under company policy.
- Comfortable talking about and addressing the topic of suicide on a day-to-day basis.
- Knowledge and compassion regarding suicide prevention.
- Ability to communicate effectively with people from varied backgrounds, both orally and in writing.
- Experience working with individuals and families who are at risk of suicide.
- Knowledge of VA benefits and services.
- Although this is not a clinical services role, a bachelor’s degree in a behavioral health or human services field is preferred.
- Military veteran, military spouse, or a Gold Star family member status is preferred.
- Previous case management experience required, especially related to suicide prevention. The case manager will have broad knowledge on the issues and challenges experienced by the Veteran community.
- Knowledge of available Veteran resources in New Hampshire is also required.
- Administer suicide risk screenings and intakes for veterans and their family members.
- Directly provide or coordinate the provision of integrated suicide prevention services, to include education, case management, emergency treatment services, peer support, benefit coordination/ enrollment, employment services, assistance with emergent needs relating to health care services, daily living services, personal financial planning and counseling, transportation services, temporary income support services, VA benefits, legal services, and child care to assist the eligible individual with issues that may contribute to the risk of suicide.

Monitor the participants progress and maintain precise and accurate documentation of case management and suicide prevention services, including client files, case notes and other record keeping as required.

Maintains and submits time sheets, mileage logs, client expenses, and other required documentation accurately and in a timely fashion.
- Attend outreach events to educate the community on program services and identify program eligible veterans.
- Provide education and training to veterans and their families on recognizing suicide risk, ways to prevent suicide, and support systems.
- Coordinate with the Peer Support Specialist and other stakeholders as appropriate to meet veteran needs.
- Develop and build partnerships with organizations and agencies that serve veterans and their families to coordinate programming and prevention services throughout NH.
- Interact with the veterans across our many programs to assist with service coordination and advocacy.
- Participate in organizational meetings and committees.
- Be available after-hours to help troubleshoot issues associated with Veterans and their families.
- May be assigned other duties as needed.

Job Summary:
The Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program (SSG Fox SPGP) provides resources toward community-based suicide prevention efforts to meet the needs of Veterans and their families through outreach, suicide prevention services, and connection to VA and community resources. In alignment with VA’s National Strategy for Preventing Veteran Suicide (2018), this program assists in further implementing a public health approach that blends community-based prevention with evidence-based clinical strategies through community efforts. The grant program is part of the Commander John Scott Hannon Veterans Mental Health Care Improvement Act of 2019, signed into law on October 17, 2020.
The case manager is responsible for SSG Fox SPGP case management services delivery to veterans and their family who are at increased risk of suicide. This includes the provision of suicide prevention services and case management to eligible veterans and families as assigned.
