      Job Summary: Case planning with goal of permanency for children either through reunification with parents, through adoption or kinGAP, or in preparation for independent living. To assure a safe and permanent outcome for every child with a focus on safety and timely permanency.

      Essential Generic Job Functions:

      • Assess safety and risk as well as protective and promotive factors and collaborate with families to create safety plans.
      • Continuously identify with the child and family their strengths, resources, and supports needed, and engage families to actively participate in the development and assessment of their service plan.
      • Work side-by-side with the foster parent to support and provide treatment to the child in care.
      • Collaborate with parent advocates to encourage and support parents in taking steps to address personal and/or family challenges so that their children can safely return home.
      • Engage parents and foster families in concurrent planning from the beginning of the case, and work with foster parents toward adoption or KinGAP as appropriate based on each child’s permanency goal.
      • Coordinate with children’s caregivers, schools, medical, mental health and other service providers, and support networks to enhance children’s well-being.
      • Have ongoing collateral contact with the Sociotherapist and all service providers to monitor progress and efficacy.
      • Complete FASPS, Psychosocial, Adoption Home studies, Kinship Homestudies, court reports
      • Attend Family Team Conferences (72 hour, 30 days, 6 month, Trial and Final discharge, placement disruptions)
      • Develop visiting plan with family and provide feedback, assess and teach parenting skills
      • Assess service needs and assist family in creation of service plan
      • Prepare, review and edit adoption summaries.
      • Prepare individual IL monthly contacts and documentation
      • Crisis Intervention as needed
      • Assist with hospitalization and discharge planning following hospitalization
      • Coordinate and facilitate family and sibling visitation/meetings. Supervise as required by the service plan. Conduct home visits to parents, foster parents and discharge resources
      • Maintain Progress Notes, FASP,, plan amendments within the required timeframe.
      • Attend all court proceedings in a timely manner with prepared court reports and /or permanency reports.
      • Assist homefinding in the recertification of foster homes as needed
      • Complete Special/Exceptional Rate Requests and Reauthorization as necessary
      • Provide support groups for families as needed
      • Implement evidence-based models with foster parents, parents, discharge resources and youth.
      • Attend weekly team meetings, consultation and supervision.
      • Provide extensive outreach and advocacy services.
      • Coordinate with other agency and community based programs such as Health Homes, Behavioral Health and ABC.
      • Provide family planning counseling to youth 12 years and older and document discussion with foster parents
      • Participate in all trainings including training for evidence-based models and other trainings in support of model implementation.
      • Work to identify family resources and support for youth.
      • Work with Youth skills coach to develop and teach youth appropriate skills.
      • Any other duties assigned

      Knowledge, Education and Skills Required:

      • MA required, MSW preferred

      Knowledge, Education and Skills Preferred:

      • Knowledge of community resources and ability to effectively negotiate systems for multi-problem families.
      • Experience with drug treatment populations preferred
      • Typing skills and computer literacy required
      • Excellent interpersonal skills
      • Spanish/English preferred

      Required Certification/Licensing:

      • Drivers’ license

      Physical Requirement:

      • While performing the responsibilities of the job, the employee is required to perform light recreational activities (i.e. walking, swimming or playing basketball)
      • While performing the responsibilities of the job, the employee is required to frequently communicate with clients who have inquiries about services that you are providing. Must be able to exchange accurate information in these situations.

      Salary Range: $56,000 - $63,000
