      What to Expect (Job Responsibilities):
      - Provide clerical and administrative support for Care Management in the Population Health Division
      - Monitor, collect, and enter various forms of data while assisting nursing functions
      - Communicate effectively with health services recipients and coordinate with providers and stakeholders
      - Assist with data collection and preparation of medical records for multifunctional processes
      - Support nursing assignments and assist with the creation and implementation of trainings

      What is Required (Qualifications):
      - High school diploma or GED required; Associate degree in Health or Human Services preferred
      - Minimum of six (6) months experience in a healthcare field required
      - Certified Medical Assistant (AAMA) certification
      - Proficient knowledge in healthcare and medical terminology
      - Ability to maintain confidentiality and establish effective working relationships

      How to Stand Out (Preferred Qualifications):
      - Experience with community-based programs and services related to physical health, mental health, and substance use
      - Strong skills in using personal computers and related software applications
      - Exceptional communication skills in both oral and written formats
