- To provide pastoral and spiritual care for patients referred from within the practice.
- To work with patients and staff in both one to one and group settings and be willing to undertake home visits if required.
- To be an active member of the practice team, taking part in relevant practice team meetings and taking a lead in pastoral and spiritual care for staff.
- To seek out and use appropriate resources and signposting to support the wellbeing and holistic care of patients and staff.
- To ensure that record keeping is up to date and accurate.

Job summary
We are seeking a compassionate, reflectiveChaplain to join our multidisciplinary team in a primary care setting. This is a two-year fixed term post offering spiritual and emotional support to patients, families, and staff within the GP practice. The successful candidate will work alongside clinical and non-clinical colleagues to contribute to holistic patient care. The current post is for between15- 20 hours per week.

Key Responsibilities of the role
Provide one-to-one pastoral and spiritual support to patients and their families, including those dealing with loss, chronic illness, or difficult diagnoses
Support staff well-being through listening services and presence-based care
Facilitate access to wider spiritual or faith-based resources for patients when appropriate
Contribute to patient-centred care planning as part of the wider practice team
Champion inclusive, respectful care for individuals of all faiths and none
