      Salary Range: $35.00 - $36.00 Hourly

      Description

      The Community Nurse is the leader and responsible for monitoring Home-health, Hospice and incident report communication and follow ups then given to Health and Wellness Director for review for need for state report and final to Executive Director. The Community Nurse is involved in residents care, interventions and recommendations when there is a significant change of condition. The Community Nurse is also responsible for the monthly weights, wound weekly assessment and documentation and weekly hospice assessments with documentation. The Community Nurse is to assist the Health and Wellness Director with tasks and in her absence, share the responsibilities for the daily care of residents at the property with the Health and Wellness Director and Care Managers.

      Qualifications


      Education: Must 18 years of age or older. Licensed Vocational or Registered Nurse in the applicable state.

      Experience: Prefer one (1) year experience in a health care environment, working with cognitively impaired older adults.

      Mathematical Skills: Ability to perform mathematical calculations consistent with medication delivery.

      Reasoning Skills: Ability to work patiently with cognitively impaired individuals. Ability to analyze stressful situations and conceive creative courses of action.

      Oral/Written

      Communication Skills Ability to read, write, and speak English. Ability to explain work duties to staff. Ability to follow oral and written directions. Ability to communicate effectively with physicians and other health care professionals. Ability to communicate effectively with residents and their families.

      Equipment Used: Standard household equipment (dishwasher, iron, vacuum cleaner, washer, dryer). Basic office equipment including computer.

      Physical Effort: Be mobile and able to perform physical requirements of the job. Ability to assist cognitively impaired people with activities of daily living. Good physical health verified by a health screening, including a chest x-ray or an intradermal test result not more than 6 months prior or seven (7) days after employment.

      Personal Characteristics: Displays sincere compassion toward cognitively impaired older adults. Demonstrates genuine concern for the physical and emotional needs of older people and their families.

      Working Conditions: Air-conditioned, well-lighted health care environment.
