What to Expect (Job Responsibilities):
- Coordinate individualized services and supports as per the Individualized Family Service Plan (IFSP) while ensuring effective communication between families and service providers
- Advocate for families to help them access resources and navigate systems like SSI and Medicaid
- Develop and monitor IFSPs by coordinating meetings, gathering assessments, and tracking progress
- Assist with outreach, screenings, and interdisciplinary assessments to support community needs
- Maintain accurate client records and ensure compliance with administrative requirements
What is Required (Qualifications):
- A valid Colorado Driver’s license
- Excellent computer skills, particularly in Microsoft Office Suite
- A criminal and motor vehicle record that meets company standards
- Verified legal status to work in the United States
- Minimum of a BA/BS in a relevant field such as education, psychology, or sociology, or a combination of education and experience in the field of developmental disabilities
How to Stand Out (Preferred Qualifications):
- Five years of experience in the field of developmental disabilities
- Experience working in human services as a case manager or direct service worker is a plus
