Job Summary
The Community Health Worker - Population Health Coordinator is responsible for implementing and managing the population health management programs and quality improvement initiatives at the respective clinic site.
Education - Bachelor's Degree in Health Education, Public Health, Social Work or or related field. Four (4) years of educational, counseling, or teaching experience in a health care setting may be substituted year for year for the education requirement.
Experience - Two (2) years clinical experience with direct patient care.
Other Requirement(s) - Knowledgeable in clinic data analysis and project management in healthcare. Familiarity with Epic Electronic health record. Familiarity with Community health plans and value based payments methods. Proficiency with Word, Excel, Database, Spreadsheet.About the company
