  JOB DESCRIPTION:

  SUMMARY OF POSITION
  Assists Clinic Outpatient and Access Services staff in engaging clients in treatment services, screening and following up with high-risk clients, assisting with care coordination and life skills as needed during transition to other services, and connecting clients to necessary resources.
  
  ESSENTIAL DUTIES & RESPONSIBILITIES
  

  • Assists Clinic Outpatient and Access Services staff in engaging clients in treatment services.
  • Assists with screening and following up with high-risk clients,
  • Assists with care coordination and life skills as needed during transition to other services,
  • Connects clients to necessary resources.
  • Understands how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model.
  • Performs all required job duties and responsibilities in accordance to CCBHC Standards.

  KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Knowledge of community geographical awareness and of community and agency resources
  • Skill in customer service techniques.
    

  
  QUALIFICATIONS
  Education Level
  Associate degree required, Bachelor’s degree preferred.
  
  Years of Experience
  At least two years of direct service experience working or volunteering in a human services organization is preferred.

  Pay Range:

  $19.80--$23.00
