- Assist the care management team with communicating transition of care services for members; obtaining medical records, processing referrals, schedule provider appointments and arrange transportation.
- Review and process referral cases to health plan programs and outpatient services through use of company-based systems to multiple programs.
- Access multiple hospital-based electronic systems to review and upload pertinent information to the outpatient care management clinical staff.
- Complete tasks generated by outpatient care management team clinical staff, including, mailings, provider appointments, and transportation.
- Documentation of completed tasks in approved computer systems and templates.
- Data entry tracking of referrals for care management teams.
- Assist members with telephonic inquiries and connect to the appropriate personnel.
- Perform other duties as assigned by clinical staff.
- Teamwork is essential to assist all members of the care management team.
- Effective communication with stakeholders, clinical staff, and members.
- High School Diploma / GED OR equivalent years of experience
- 2+ years of office/clerical experience working with customers
- Experience with computers including Windows based programs Microsoft Word, Microsoft Excel and Microsoft Outlook
- Ability to come in to office at least once a week and for Team meetings as needed
- Proficient typing skills
- Must be 18 years of age OR older
- Ability to work full-time, Monday - Friday between 8:00am – 5:00pm PST including the flexibility to work occasional overtime given the business need
- Experience working within the health care Industry and with health care insurance
- Experience working in a hospital, physician's office or medical clinic setting
- Knowledge of EMR systems
- Experience working in a call center
- Reside within 50 miles of 2720 N Tenaya Way Las Vegas, NV 89128
- Ability to keep all company sensitive documents secure (if applicable)
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
- Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

The Clinical Administrative Coordinator assists the Medical Management team with communicating transition of care services for members; obtaining medical records, processing referrals and scheduling appointments and transportation.
Receiving referrals from multiple sources as well as e-mails from facilities and assigning it to the appropriate team. Check eligibility of the referral for outpatient case management programs and assign them to SOP. Send out appropriate mailing for each team to members and providers. Provide phone coverage for the Continuity of Care programs. Monitor and tracking of referrals. Track declines and reasons and update management on any concerns/trends. Build cases for members and assign it to proper personnel (LSW or Case Management). Reports to Manager. Weekly meetings held via Team meeting and sometimes they will be in the office. Monthly 1:1. Required to come in to office a minimum of once a week to pick up supplies, check box.
This position is full time, Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules between our normal business hours of 8:00am – 5:00pm PST. It may be necessary, given the business need, to work occasional overtime.
Our office is located at 2720 N Tenaya Way Las Vegas, NV 89128. Employees will be required to come in to the office at least once a week and for team meetings as needed.
We offer 6 weeks of paid training. The hours during training will be 8:00am to 5:00pm PST, Monday - Friday. First day training in office, then continues remote. Training will be conducted virtually from your home.

This is a high volume, customer service environment. You'll need to be efficient, productive and thorough dealing with our members over the phone. You also must be able to work a flexible schedule

Nevada Residents Only: The hourly range for this is $16.54 - $32.55 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
