What to Expect (Job Responsibilities):
- Serve as a liaison between healthcare services and members, coordinating care and support
- Prepare members for telehealth visits, ensuring they feel comfortable and at ease
- Process and coordinate palliative and hospice care orders, ensuring timely support for members
- Educate members on existing clinical programs and assist with care coordination issues
- Follow up with members post-hospital discharge to ensure smooth transitions and adherence to care plans
What is Required (Qualifications):
- Minimum of 2 years of outpatient experience in healthcare settings such as Palliative, Hospice, Oncology, Dialysis Centers, or Behavioral Health
- Must be comfortable performing care coordination for the Medicare population across multiple states
- Effective communication skills to interact with various stakeholders in a member’s care
- Ability to engage with members over video and phone, showing compassion and empathy
- Familiarity with electronic health records and various computer platforms
How to Stand Out (Preferred Qualifications):
- MA Certification (highly preferred), including expired certifications
- Fully bilingual in English and Spanish
- At least 9 months of experience in a current role within the employer
- Significant MA experience or background, with documentation such as diplomas and transcripts as needed
- Entrepreneurial spirit with strong teamwork skills
