      Program Description

      The Intensive Case Manager (ICM) is part of Felton’s Geriatric Mental Health Model, which offers mental health services to older adults with severe psychiatric disorders and includes a continuum of care, including treatment of dual diagnosis and homeless individuals. Under contract with the Department of Public Health – Behavioral Health Services (BHS), the ICM program provides city-wide services to older adults needing a high level of care. The agency is committed to implementing Evidence-Based Practices and behavioral health research into its community-based mental health services.

      Job Description

      Under the supervision of the Program Manager, this position participates in a multidisciplinary team serving older adults, aged 60 and over, with severe psychiatric disorders and co-occurring disorders, providing comprehensive and integrated treatment services, including mental health and substance abuse treatment, case management, and other service connections. It employs an inter-agency, multidisciplinary approach to provide comprehensive wellness/recovery-based services. Referrals for the ICM caseload come from BHS and the caseload is capped at 16 clients to provide the intensive services needed. Once stabilized, clients are stepped down to Geriatric Outpatient Services at Felton or other clinics. The ICM clinician evaluates the needs of clients, and develops treatment plans, often along with the medication support team. Services may include individual and group therapy, crisis intervention, case management, client advocacy, prevention, and education. The ICM clinician maintains accurate detailed observation and progress records for electronic billing data entry. Additional duties as needed. This position may also include a partial caseload of clients in the Geriatric Outpatient Services level, depending on the need.

      Program Specific Responsibilities:

      • Works closely with an interdisciplinary team of clinical case managers, psychiatric nurse practitioners, psychiatrists, and community specialists (peer counselors) to provide intensive services to older adults in crisis.
      • Performs intakes and comprehensive psychiatric assessments for older adults with psychiatric and dual diagnosis disabilities in a Felton clinic or in their homes utilizing both psychiatric interviewing and DSM V-TR/WHODAS measures of both mental health symptoms and functional impairments
      • Provides crisis intervention and management to prevent hospitalization, and if necessary, facilitates involuntary hospitalization.
      • Provides individual and group therapy to stabilize clients and prevent psychiatric de-compensation.
      • Provides case management services, including evaluations, service plan development, assisting clients in accessing housing, financial, medical, and other needed services. Coordinates with other service providers.
      • Maintains thorough clinical records after each client contact or service, in accordance with mandated standards of San Francisco Community Mental Health Services and funders including Medi-Cal/Medicare
      • Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
      • Ongoing training by participating in FSA’s training opportunities, including developing specializations in evidence-based treatments and practices.
      • Other duties and responsibilities that may be assigned from time to time

      Minimum Qualifications:

      • Masters degree or higher in Social Work or other Professional Counseling Programs, including MFTs/APCCs
      • Registered with active license with BBS, including intern level ASWs, AMFTs, and APCCs
      • Experience in case management and mental health treatment for at-risk older adults, including co-occurring disorders (master’s level internship experience considered) preferred.
      • Moderate to comprehensive understanding of Specialty Mental Health Services under the Department of Health Care services definitions in addition to skills in assessment, treatment planning, and mental health services pertaining to individuals living with serious mental illness
      • Excellent communication skills both in written and verbal formats
      • Knowledgeable about and consultation oriented towards client risks for suicidality/violence, abuse reporting, dual role conflicts, and other ethical/legal obligations as they arise
      • Solution focused care planning and conceptualization skills within multi-disciplinary team and with external agency partners and individuals
      • Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds and who often are underserved by traditional care models
      • Ability to effectively diagnose and make clinical rationales for psychiatric care utilizing the DSM 5-TR and other measures including social determinants of health and functional impairment screenings where appropriate
      • Valid CA Driver’s License preferred, ability to travel across SF city to meet clients required
      • Ability to work well under pressure while meeting multiple different types of deadlines
      • Computer skills proficiency in applications for electronic health care records, Zoom, Box, Microsoft/Outlook applications, and other technologies as needed for the role
      • Ability to be a team player with a welcoming attitude
