      Job summary

      The clinical coder plays a role in ensuring the accurate entry and maintenance of data within patient records.

      This position requires a well organised individual with a good knowledge of medical terminology. They must have strong organisation skills and be able to work independently whilst liaising with healthcare professionals where needed. Knowledge of Systmone is advantageous.

      Main duties of the job

      The primary responsibilities of the role include reviewing, summarising and coding patient information form medical records in both electronic and paper format while adhering to national clinical coding standards and practice protocols.

      They will also be responsible for the patient recall system for long term conditions. Proactively identify patients who are due or overdue health checks and reviews ensuring that they are contacted and booked.

      We are a small but growing practice with over 6500 patients. Located in the north of the vibrant university city of Lincoln. Currently our administration team is a small group of 5 who work very closely and support each other.

      Job description

      Job responsibilities

      The following are the core responsibilities of the Clinical Coder. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

      a. Accurately clinically coding data on the clinical system into the patient record

      b. Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed action

      c. Inputting and extracting data as required to support practice reports

      d. Filing and storing records as required

      e. Photocopying documentation as required

      f. Actioning all incoming correspondence, emails and mail in a timely manner

      g. Scanning patient related documentation and attaching scanned documents to patients healthcare records

      h. Inputting data into patients healthcare records as necessary

      i. Carrying out system searches as requested

      j. Lead on coordinating patient recalls for long-term condition reviews

      k. Proactively identify patients overdue for health checks or reviews ensuring they are booked appropriately.

      l. Maintaining a clean, tidy, effective working area at all times

      m. Supporting all clinical staff with general administrative tasks as requested
