Summarising medical records and accurately entering relevant information into patient records.

Ensuring clinical information is appropriately coded within EMIS; prior experience with EMIS coding is essential.

Managing incoming documents and correspondence as part of the daily workflow, ensuring timely and accurate processing.

Supporting the clinical team by maintaining up-to-date, well-organised patient records.

Assess and sort patient records and correspondence in preparation for summarising.

Review both paper and electronic medical records and produce an accurate, relevant summary of each patients medical history.

Read code clinical information into the practices EMIS Web system.

Audit data collection standards within the practice as required.

Monitor the progress of record summarising in line with practice targets and timelines.

File and retrieve documentation as needed.

Carry out other routine administrative duties and data input tasks as required.

Process all practice correspondence (paper, electronic, and email), ensuring it is correctly attached to the appropriate patient record.

Accurately read code diagnoses, procedures, and investigations, and take relevant actions such as arranging patient appointments or completing referrals.

Identify complex or higher-risk informationsuch as safeguarding concernsthat require clinical input, and escalate in accordance with practice protocols.

Take responsibility for GP2GP electronic record transfers.

Manage the Document Management System, ensuring all incoming documents are processed accurately and promptly.

Job Summary
Summarise and Read Code relevant information from patients clinical records into the Practice computer system, working strictly in accordance with the Practice notes summarising protocol.
Provide general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.

Culling and sorting patient records and letters in preparation for summarising
Reviewing medical records and producing an accurate summary of the patients medical history.
Read Coding the information into the Practice clinical system

- Monitoring patient call and recall systems by Liaising with other staff to ensure review clinics are booked
- Coding & merging from GP2GP
Collation of Information
Support the Practice team in achievement of Enhanced Service and Quality Points targets by ensuring the following :
Collation of information using EMIS medical information system.
Assist with the management of disease registers.
Supporting GPs & mangers in prescribing Audits
Taking action on CEG Prevalence report
Assisting GP in CKD audit and Electronic referrals
Administration
Scanning All incoming clinical letters and faxes in patient notes
Checking & Coding e-Documents & All discharge summaries and Clinical Documents & Faxes
Maintaining RIP(Rest in Peace) Register, Coding & Taking appropriate actions
Checking Practice generic emails and taking action

To participate in training sessions to ensure administrative and computer skills are kept up to date with current technology and practice.
Attending all clinical meetings
