      Job summary

      This clinical coding post sits within the administration team and is based at Collingwood site, however, staff may be requested to work at other sites on occasions.

      To be responsible for the read coding of relevant information into patients electronic healthcare records on the practice's clinical system, adhering to the practice policy

      In addition, the post holder will support the multi-disciplinary team, providing administrative support as required, in line with the needs of the organisation. The post holder will also support the management team in promoting ED&I, SHEF, Quality & CI, Confidentiality, Collaborative Working, Service Delivery, Learning and Development and carry out other duties as directed by the management team.

      Main duties of the job

      The postholder must be able to accurately translate information documented in patient medical records or hospital correspondence into coded data and enter onto clinical system.

      Collingwood Health Group is based in North Shields in the North of England approximately 9 miles from Newcastle. We are based over 3 sites with a patient population of around 24500.

      Job description

      Job responsibilities

      The following are the core responsibilities of the clinical coder. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.

      a. Accurately read and extract clinical information, subsequently coding this data on the clinical system into the patient record

      b. Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed action.

      c. Inputting and extracting data as required to support practice reports.

      d. Photocopying documentation as required.

      e. Actioning all incoming correspondence, emails and mail in a timely manner

      f. Scanning patient related documentation and attaching scanned documents to patients healthcare records

      g. Inputting data into patients healthcare records as necessary

      h. Answering incoming telephone calls, transferring calls or dealing with the callers request appropriately

      i. Carrying out system searches as requested.

      j. Maintaining a clean, tidy, effective working area at all times

      k. Supporting all clinical staff with general administrative tasks as requested

      l. Have a good awareness of QoF and the importance of accurate information.

      m. Review and summarise the medical records of patients newly registered with the practice.
