      Summary :

      The Clinical Dietitian assesses nutritional status and develops and implements a comprehensive nutrition care plan for patients. Participates in the interdisciplinary team providing care coordination, education, and support. Provides nutritional counseling, education, and symptom management to clients in classes or individually in the outpatient setting. This is newly created outpatient position with the ThedaCare Surgery Optimization & Coordination Center (TSOCC) and Nutrition & Diabetes Education Departments.

      Job Description:

      KEY ACCOUNTABILITIES:

      1. Develops, implements, monitors, and coordinates nutritional care plan based on assessment to client's needs with other health professionals.

      2. Completes nutrition focused physical exams

      3. Counsels individuals and family members on nutritional principles, dietary plans, food selection, and economics of nutrition. Adapts plans to individual client's needs, lifestyle, and cultural eating habits.

      4. Provides follow-up and makes appropriate referrals based on needs of individual.

      5. Documents assessments, recommendations, and plan of care in EPIC. Documents in EPIC care plans and education interventions and goals.

      6. Recommends home care nutritional needs.

      7. Interprets, evaluates, and utilizes current research to achieve effective nutritional care.

      8. Utilizes the Nutrition Care Manuel from the Academy of Nutrition and Dietetics as a resource for educational materials.

      9. Participates in process improvement work to support delivery of best care to patient in the acute care setting. 10. Serves as a preceptor for dietetic interns.

      QUALIFICATIONS:

      • Registered Dietitian • Certified Dietitian within 6 months of hire.

      PHYSICAL DEMANDS:

      • Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance

      • Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

      WORK ENVIRONMENT

      • Climate controlled office setting with daily movement throughout the facility

      • Interaction with department members and other healthcare providers
