      About our Unit:

      3West is a 48-bed specialized unit comprised of rehab patients.

      We are a skilled group of rehabilitation healthcare professionals who strive for excellence in providing compassionate, patient-centered care. Our goal-focused therapy guides our patients in their journey to independence and reintegration into community. Our team works diligently towards improving the physical, cognitive and communication problems caused by an injury or illness. We work with our patients to improve their quality of life and support their desire to return to meaningful activities.

      Our patient’s success is optimized by self-motivation, active participation and family support.

      Role Summary:

      The successful candidate will be a highly motivated, agile and adaptable professional, with advanced clinical and critical thinking skills. As an integral member of the 3West leadership Quad, along with the Patient Care Manager, Clinical Educator, and Charge Nurse, you will have the opportunity to support staff to provide the highest quality patient and family-centred care, while optimizing staff engagement and satisfaction.

      The Clinical Leader is accountable for overseeing patient care and flow on the unit, including implementing processes that drive excellence in patient safety and quality outcomes. You will provide mentorship and education at the bedside and role model teamwork, communication, autonomous clinical practice, critical thinking, best clinical practices, quality care and patient safety standards.

      Key Qualifications, Skills and Abilities:

      • Current membership (in good standing) with the College of Nurses of Ontario is required
      • Baccalaureate degree in Nursing, or equivalent experience paired with evidence of degree in progress (or commitment to commence the degree process in the near future) is required
      • Masters degree is an asset
      • A minimum of three to five years clinical experience with a rehab and geriatric patient populations, as well as patients with complex medical care needs, is required
      • Current BCLS required, and GPA certification is an asset
      • Experience using Epic and Microsoft Office (Excel, Word, PowerPoint) is an asset
      • FIM certification or to be FIM certified within 6 months of hire
      • Minimum five years clinical experience with a demonstrated understanding of technical and/or clinical information related to the designated Health System noted above.
      • Previous experience in a nursing leadership role is preferred
      • Clinical and leadership skills, which develop and empower members of the health care team in autonomous decision-making to achieve desired outcomes
      • Strong critical thinking, decision making, problem solving and conflict resolution skills
      • Excellent communication and facilitation skills – both oral and written
      • Demonstrated excellent interpersonal skills and the ability to work collaboratively with all members of the interprofessional care team
      • Ability to coach, mentor and provide feedback while fostering a positive work culture
      • Demonstrated commitment to patient and family-centered care including process and outcome evaluation
      • Ability to function effectively in a fast-paced environment
      • Ability to coordinate and delegate duties
      • Ability to manage workload with many competing priorities
      • Ability to provide leadership for unit-based projects and accomplish established goals
      • Commitment to evidence-based practice and quality improvement
      • Working knowledge of change process/theory and demonstrated ability to manage change within a team
      • Excellent work record and attendance is essential

      Accountabilities and Responsibilities:

      • Work collaboratively with the unit leadership to coordinate, manage, organize and prioritize day-to-day patient care activities, staffing and resource utilization for the unit
      • Management of operational issues with a focus on issues related to capacity, patient relations, quality & risk and safety.
      • Be responsive to the fluctuating needs and demands for operations and capacity management support, including collaboration with Rehab Admission Coordinator to facilitate admissions within a timely manner
      • Provide leadership to the interprofessional team to ensure integrated and comprehensive care planning occurs in order to achieve safe and effective patient care, including leading team meetings, rounds, and huddles
      • Monitor and evaluate patients’ progress against the plan of care, and suggest changes based on fluctuating needs and priorities
      • Serve as an expert clinical resource for the unit and support staff at the bedside for complex situations, as required
      • Act as the primary liaison for physicians, nurse practitioners, the interprofessional team, families and community resources
      • Support the accountability of team members to the Patient Care Manager with regards to patient care, professional conduct and performance
      • Regularly complete chart reviews, audits and follow-ups to ensure adherence and compliance to standards of care Round, monitor, and address patient and family satisfaction with care, health and safety issues, and the physical environment in order to meet our standards for best practices
      • Identify and communicate risks, and develop/implement initiatives with the team and leadership in order to optimize patient and team safety
      • Assist in the development of effective interpersonal relationships within the team through coaching, mentoring and facilitating difficult conversations
      • Support patient flow (including admissions, discharges and transfers) of patients through a transparent, equitable and standardized process
      • Promote and implement clinical practice standards and evidence-based best practice to improve outcomes
      • Support the team to provide patient & family education using available tools and resources
      • Participate in hiring, orientation and performance evaluation as requested
      • Lead/facilitate unit-based committees or working groups to achieve unit goals & objectives
      • Participate in unit, program and hospital-wide initiatives that support THP’s strategic direction
      • Utilize new technology and evaluates its impact on staff, patients, families and overall best practices
