      POSITION SUMMARY
      Serve as a resource representing Health First and Clinical Outpatient Wellness Services (OWS), in particular Hospice and Home Care services, to ensure quality, competitive value and customer delight. Participate in support groups, community educational offerings and related meetings to promote Hospice and Home Care OWS. Partner with subject matter experts when providing oversight on educational activities, including collection and analysis of education/clinical-related research data; development of specific educational programs, and promoting goals through education. Communicate, monitor and report results of outreach and educational activities.

      PRIMARY ACCOUNTABILITIES
      1. Promote and educate on approved service line topics to hospitals, case managers, discharge planners, physicians, and various organizations about OWS Hospice and/or Home Care services
      2. Serve as liaison between the organization and physicians to ensure continuity of patient care
      3. Establish self as an integrated partner with case management teams at designated Health First hospitals, serving as an ongoing resource regarding early identification of patients who are medically appropriate, admission criteria, benefits and quality of service, while continually educating staff on importance of such topics
      4. Attend appropriate meetings, internal and external, including weekly case conferences as needed, and to problem solve any barriers to business, or when regulatory considerations change. Escalate potential conflicts between internal stakeholders to expedient solutions in the mutual best interest of Health First
      5. Assist OWS Management in defining tactics and business strategy, achieving goals, and financial and other business objectives
      6. Obtain and organize community related information; maintain referral sources and provide input on customer opportunities, responding promptly as appropriate
      7. Ensure all PHI information is protected per HIPPA guidelines and department processes
      8. Coordinate case management with appropriate representatives to ensure eligibility for services to facilitate payment of claims
      9. Attend all OWS clinical meetings and participate on committees as needed or requested; follow all Health First policies, procedures, and requirements

      MINIMUM QUALIFICATIONS
      • Education: Associates Degree
      • Work Experience: Three (3) years hospice, home care or other outpatient related work
      • Licensure: Varies – a nurse or therapist must show proof of current State of FL; non clinical does not hold a license
      • Certification: None required; Satisfy AHCA Level 2 requirement if applicable, e.g., Hospice - must satisfy AHCA Level 2 requirement
      • Knowledge/Skills/Abilities:
      • Microsoft Office products, including PowerPoint; various software platforms related to specific service line or scope of role
      • Presentation and facilitation
      • Communication – written, auditory and verbal
      • Critical thinking; Problem solving
      • Interpersonal; relate and build relationships with customers (patients, family members, physicians, Health First Associates, Community Leaders
      • Valid driver’s license, automobile insurance and reliable transportation
      • Knowledgeable of regulatory guidelines

      • Education: Bachelor’s or Master’s

      • Knowledge/Skills/Abilities:
      o Regulatory guidelines related to specific line of service, e.g., Medicare/JCAHO Regulations

      PHYSICAL REQUIREMENTS (ADA, Travel, Environment, Noise)
      Varied physical requirements. Frequently sit, stand, walk, bend, and stoop; Hand-eye coordination, vision coordination of close to far range; Occasionally carry or move up to 20 lbs . Travel approximately 80% of time within Brevard County and service areas, Occasional work weekends and evenings
