      Job summary

      The Community Advance Care Planning in Care Home Team provides support to the Shropshire, Telford and Wrekin Care Homes through an inspiring and supportive multi-disciplinary team approach. This is achieved by the delivery of a rolling and evolving programme of initiatives and patient case management.

      The Team develops proactive Care Plans with a particular focus on frailty, including ReSPECT, Advance Care Plan (ACP) and Anticipatory Care Plan (ANCP).

      An exciting opportunity has arisen to recruit a part-time Band 6 Senior Nurse who can support the team to further develop and deliver the service within Shropshire, Telford and Wrekin.

      We are looking for a highly motivated and inspiring nurse with significant experience across a broad range of clinical areas, with excellent communication and demonstrable leadership potential.

      You will need to be committed to delivering high standards of care, being a role model to others and be experienced in supporting change management and service development.

      Main duties of the job

      To provide specialist nursing interventions, pro-active case management and care planning for the Advance Care Planning in Care Homes Team who will be delivering clinical care planning for residents of Care Homes (Residential and Nursing homes) in Shropshire, Telford and Wrekin.

      To enhance the delivery of education to staff working within the Care Home setting, in order to support them working towards a proactive model of care for residents within their care setting.

      The post holder will facilitate the avoidance of inappropriate hospital admissions by formulating and case managing Advance Care Plans (ACP) and Anticipatory care plans (ANCP) alongside the MDT. The teamwork with a vision to enhance existing procedures, in line with current national guidance to drive improvements in quality of care.

      • Level 1 Adult Nurse Registration with Nursing and Midwifery Council (NMC).
      • Diploma/Degree Specialist Practice District Nursing or alternative post registration qualifications in a relevant discipline or portfolio of significant relevant experience.
      • Community Practitioner Nurse Prescribing (V100 or V150) or willing to work towards this within 18 months of appointment.
      • Recognised physical assessment qualification or willingness to work towards this within 12 months of appointment.
