Advanced Clinical Support: Provide expert nursing input and assessments for individuals with complex health needs within a social care environment, ensuring care plans reflect current clinical best practices.

- Clinical experience including Tracheostomies, ventilators, stoma bags etc
- Ideally, previous knowledge and/or experience in working in a complex care setting or community setting
- Excellent communication skills and ability to be able to train clinical and non-clinical staff
- Submit clinical reports and updates to CCGs regarding their packages as per contractual agreements
- Maintain clinical updates and evidence of PREP (Post Registration Educational Portfolio)
- Ideally have leadership skills/experience
- Driving licence and use of a vehicle for work

Job summary
Due to our expansion in our Clinical Services we are seeking to appoint an enthusiastic and experienced individual to AdultCommunity Nurse. This is a fantastic opportunity to utilise your skills and provide exceptional healthcare to our clients and their families. This is a unique opportunity to create and establish our Clinical services in the area.
Main duties of the job
Youll be working with our Carers and complex care patients in their own homes within the community, ensuring that Carers are suitably trained and competent to deliver the care needed. Youll assess the risks and needs of the patient to devise an individual holistic detailed electronic care plans and that they are regularly evaluated to ensure that patients receive supervised evidence based nursing care in accordance with local contracts.
