      Location: Bromley but may require working from the Sydenham site

      Hours: Part Time 22.5 hours per week Maternity Cover fixed term or secondment opportunity.

      Salary: £31,129-£35,126 (inclusive of HCA, 22.5 hours per week)

      The Bromley Hospital@ Home Service is a dynamic, collaborative and integrated model of care that intends to deliver a range of interventions, including remote monitoring in the community. It aims to help patients avoid hospital admission, enable early discharge from hospital and to remain at home through the use of short-term support and intervention. St Christophers is part of this new service (The Hospital@ Home Palliative Team), and is looking to recruit a CNS/CPS to cover maternity leave within the team

      What will you be doing? You will work across organisations to support people to remain in their own homes rather than attending hospital or facilitating a rapid discharge home from hospital when appropriate using the Hospital@home model as a basis for care provision. The Palliative Pathway in this innovative service, supports people identified as reaching the end of their life. You will be an integral part of the Bromley Hospital@Home Team and St Christophers Community Palliative Care Team. You will also work closely with The PRUH Acute Frailty Unit, St Christophers In Reach Nurse and PRUH Inpatient Palliative Care Team.
