- Performs comprehensive patient/family history and assessment
- Co-develops care plans with patients/families in collaboration with the interdisciplinary team
- Manages complex patient and family needs and advocates for patients and families
- Shares information and coordinates care to provide seamless care
- Models an interprofessional team practice approach
- Identifies gaps in care and quality at the individual, organizational and system levels
- Registered Nurse in good standing with the College of Nurses of Ontario
- Master's in Nursing Degree is required
- Expertise in providing care to the older adult and to patients with responsive behaviors
- Gentle Persuasion Approach in Dementia Care certification and Coach preferred (or willingness to become a certified coach)
- Knowledge of P.I.E.C.E.S, U-First, CPI, SMG or willingness to receive such training
- Ability to implement best practices in dementia care, delirium, mental health issues with an ability to deliver holistic patient care
- Ability to collaborate effectively with other members of the interprofessional care teams, patients, family members, Physicians and community partners
- Demonstrated ability to work independently with minimal supervision
- Demonstrated understanding and compliance with patient and staff safety policies and procedures that foster a safe and inclusive environment for all
- Models behavior that aligns with THP values
- Experience in gerontological nursing is required
- Experience in wound care is an asset
- Experience in working with patients with Behaviors (BPSD) is required
- Client-centered care philosophy
- Excellent organizational and problem-solving skills
- Excellent oral and written communication skills, and group presentation skills
- Able to prioritize tasks and manage time well
- Able to work independently and collaboratively as a member of an interprofessional team
- Strong computer skills and proficient in basic Microsoft 365 applications (excel/word)
- Excellent customer relations and being consistently courteous, understanding, and respectful in interactions with clients, partner organizations and colleagues
- Ability to meet the physical demands of the job
- Satisfactory work performance and attendance required
- Valid CPR
- Assesses patients with Dementia, Delirium, responsive behaviors and other Mental health disorders
- Creates successful treatment plans with clients based on a self-management and client-directed goal setting philosophy
- Links with the most responsible Physician, Geriatrician and other health care professionals as necessary
- Delivers education sessions to increase awareness of senior friendly care, Geriatric syndromes and best practices in caring for the older adult

Reporting to the Director for Rehab, Complex Continuing Care, Transitional Services, Seniors, Palliative and Geriatrics, the CNS in Geriatrics will exemplify the qualities of the Advance Practice Nurse, including:
The Reactivation care center is a transitional space with 99 in-patient beds. All patients are Alternative Level of Care (ALC) and haae been deemed medically stable. The patient population at the RCC may include patients with responsive behaviors, Hemodialysis, palliative and convalescent. The CNS is an integral member of the IP team and will guide care planning, assessment, interventions, teaching and leadership for the entire team. Their primary focus will be on supporting patients with responsive behaviors, Mental health issues or complex Medical/Psychosocial needs.
