- The CNS/P provides specialist clinical advice and support to health care professionals within the Integrated Care Board (ICB). They will work with the clinical lead for the development of the respiratory service towards a consistent and integrated approach to respiratory care in this area.
- The CNS/P will work within the integrated respiratory team, providing virtual ward care alongside care to patients with chronic disease illness predominantly in their home, but also in clinic locations and on the acute hospital site
- To co-ordinate care and services for patients with respiratory illness to ensure that they receive the highest quality of care, support and experience within agreed protocols and allocated resources together with the clinical lead and colleagues.
- To support patents with their own health and well-being needs through the provision of appropriate information, education, advice, and support particularly to patients with respiratory illness.
- To be an expert resource who provides sound clinical advice and judgement to patients and their families, nurses, and junior doctors.
- To assist the Clinical Lead with the development, implementation, monitoring and audit of recommendations made by best practice criteria and Peer Reviews.
- Within North-East Hampshire, Farnham, and Surrey Heath area act as a specialised resource on respiratory care and Oxygen use, to patients, health care professionals and the general public.
- Manage own caseload of respiratory patients and familiarise with all aspects of the Respiratory Service and Virtual Ward SOP/Clinical Flowcharts and Escalation Procedures. Having evaluated outcomes, safely discharge patients from the service.

- Teaching / assessment programme
- Relevant specialist post basic programme in respiratory care and management
- First degree in nursing, or demonstrable experience at this level
- Completed / completing a masters degree.
- Non-Medical prescribing
- Minimum 4 years relevant clinical experience (postgraduate) in the same or related specialty
- At least 2yrs recent clinical experience in the acute or respiratory setting
- Experience in managing the acutely unwell respiratory patient in the hospital setting.
- Evidence of ability to work collaboratively.
- Previous experience in developing standards, guidelines and policies
- Previous experience in audit and evaluation
- Experience in acting as a clinical supervisor and mentor to others
- Demonstrates understanding of relevant national strategy / policy and how this relates to the service
- Experience of applying research evidence to clinical practice
- Previous community nursing experience
- Previous experience of university education or professional development role.
- Full driving licence and access to a car for use to travel across sites and perform domiciliary visits.

We are expanding our successful teams to provide Acute Respiratory Virtual Ward and we are looking for an enthusiastic and knowledgeable B7 Clinical Nurse Specialist who is committed to delivering and ensuring others deliver excellent, evidence based respiratory care. You will be part of a senior team, leading and developing nurses, physiotherapists, nursing associates, associate practitioners, physio technicians and administrative support to deliver care for people with respiratory illness in the place of choice.
The successful candidate will play an integral part in improving the standard of care for respiratory patients and will have the opportunity to work within a variety of settings. The award-winning team is based at Farnham Community Hospital and covers NE Hampshire, Farnham and Surrey Heath area. We see patients as hospital in-patients, in clinic and in their own homes. The focus is supporting people to manage their condition within the most appropriate setting. No two days are the same!
The job will require working in other designated locations across the ICB as well as the primary base. Flexibility is required across secondary care and community.
Main duties of the job
The Virtual Ward is an exciting new service offering home based care to patients who would otherwise occupy a hospital bed. The care is delivered by a specialist workforce using face to face assessments and an integrated pathway to safely support patients in their own home. Working closely with other regional experts, you will work both independently and as part of the supportive multi-disciplinary team, providing care that is underpinned by a culture of research and innovation.
The successful candidates will provide specialist respiratory intervention, as well as supporting the multidisciplinary team in assessment and development of a holistic management plan. You will be expected to perform assessment of respiratory patients with diverse presentations and complex physical and psychological conditions, developing and delivering an individualised physiotherapy treatment programme based on this. You will understand the assessment and management of multiple respiratory conditions and be aware of local and national guidelines for COPD, Bronchiectasis, Interstitial Lung Disease, Asthma and Oxygen.
