Job summary
Aldershot PCN is looking for a Clinical Pharmacist. Duties may vary from time to time without changing the general character of the post or the level of responsibility. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.
Main duties of the job
The following are the core responsibilities of the PCN Clinical Pharmacist. To work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
To liaise with the practices and, where practicable, help to standardise the medicines management process across the PCN.
To consult patients within defined levels of competence.
To receive referrals and directed patients from triage services and other clinicians.
To receive and resolve medicines queries from patients and other staff.
To support the review of medications for registered patients
To provide proactive advice and support on medicines and prescribing systems to the PCN multidisciplinary team, patients and their carers
To encourage cost-effective prescribing throughout the PCN
To provide advice and answer medication related queries from patients and staff
Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives
To support in the delivery of enhanced services including Enhanced Care in Care Homes and other service requirements on behalf of the PCN
To support improvement of the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards and clinical and prescribing guidance
About us
Salus is a GP Federation which was formed by the local GP practices within North East Hampshire and Farnham. This Includes practices located in Aldershot, Farnborough, Fleet, Farnham & Yateley. We hold several contracts supporting primary care to deliver services on behalf of those practices, helping them work better together and with other healthcare providers leading to improved patient services and developing new ones due to constant changes in the Primary Care environment.
We also assist practices with admin, operational and HR support and facilitate shared learning across localities. By engaging with the public, we also help ensure that services are designed and developed with patient and public involvement.
The staff at Salus Medical Services have a wealth of experience working in general practice and therefore a good understanding of the opportunities and challenges faced.

To actively support the signposting of patients to the correct healthcare professional
To act as the PCN central contact for all medicine related matters, establishing positive working relationships with community pharmacists, hospital pharmacists, other PCN Clinical Pharmacists and other stakeholders in the medicines supply chain to improve patient experience and manage incidents.
Provide peer support to Pharmacy Technicians and prescription administrative staff.
To review the latest guidance ensuring the practice conforms to NICE, CQC etc.
To participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events.
To undertake all mandatory training and induction programmes
To contribute to and embrace the spectrum of clinical governance.
To attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.
To contribute to public health campaigns (e.g. flu clinics) through advice or direct care
To maintain a clean, tidy, effective working area always.
You should be
The successful applicants will need to be self-motivated and capable of networking with both the PCN team as well as practice teams. Whilst also being able to work on their own.
Previous PCN/Primary Care working experience is desirable.
You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing medicines management in the NHS.
