- Patient Consultations: Conduct patient-facing clinics for chronic disease management, medication reviews, and provide expert advice on medication-related queries.
- Medicines Reconciliation: Ensure accurate and safe prescribing by reconciling medications post-hospital discharge and at other transition points

- Registered pharmacist with the General Pharmaceutical Council (GPhC)
- Postgraduate qualification in clinical pharmacy or equivalent experience.
- Independent prescribing qualification is desirable.
- Experience in a primary care setting, with a strong understanding of general practice operations.
- Excellent communication and interpersonal skills, with the ability to work effectively within a team.

Job summary
Chiswick Medical Practice, serving the community across 3 sites Grove Park, Wellesley Road, and Chiswick Health Centre, is seeking a dedicated and experienced GP Clinical Pharmacist to enhance our multidisciplinary team.
