      Main duties of the job

      As our Clinical Pharmacist, youll have a pivotal role in improving patient outcomes and supporting safe, evidence based prescribing. Working with Pharmacy Team and GP mentors, you will:

      Lead clinical medication reviews, especially for patients with complex needs and long-term conditions (e.g., COPD, asthma).

      Deliver patient-facing clinics and telephone support, providing expert medicines advice and empowering patients to manage their health.

      Support frailty care, repeat prescribing processes, and medicines reconciliation following hospital discharge.

      Take forward quality improvement projects and prescribing audits that enhance safety and effectiveness.

      Promote the implementation of national guidelines and local formulary recommendations.

      Contribute to education, training, and public health campaigns.

      • Registration with the General Pharmaceutical Council.
      • At least 2 years post qualification experience.

      • Excellent communication skills and confidence working in a multi-professional environment.

      • Independent Prescriber qualification.
