Job summary
We are looking for an enthusiastic, hardworking Foundation Pharmacist to join our team of skilled Clinical Pharmacists and Technicians to provide high quality medicines optimisation for the Trust. You will provide a clinical pharmacy service to a group of wards under the supervision of a Senior Clinical Pharmacist participating in various rotations through the ward specialities. During your rotations, you will be supported by the practice supervisor to develop your skills and knowledge in a multitude of areas. We support the development of our staff through our Education and Training Department.
Main duties of the job
If you are ready to develop your knowledge in hospital pharmacy then we can support you. You will work alongside the multidisciplinary team on matters relating to the safe, economic, effective and evidence based use of medications. With support from our Medicines Management Technicians you will ensure that patients and carers understand how to take their medicines on discharge including counselling and liaising with primary care colleagues to ensure continuity of care. Rotations include training in the Medicines Information service, developing skills in handling enquiries on a range of topics from a variety of enquirers. Whilst on the ward Pharmacists are expected to implement the Trust Medicines Formulary and guide medical staff on adherence to Trust guidelines. All staff are expected to support and promote adherence to antimicrobial guidelines and safe and appropriate prescribing within the trust.
