Job summary
Willows Health (NHS) has exciting and unique opportunities for Clinical Pharmacist to join our leading Pharmacy department in primary care. The successful candidate will focus on delivering complex clinical medication reviews for patients with chronic diseases. They will work collaboratively with GPs, nurses, and other healthcare professionals to optimise medication use, improve patient outcomes, and support integrated chronic disease management.
Main duties of the job
Conduct comprehensive, complex clinical medication reviews for patients with multiple chronic conditions such as diabetes, hypertension, COPD, asthma, heart failure, and others.
Assess and optimise pharmacotherapy, ensuring evidence-based and patient-centred care aligned with current clinical guidelines (e.g., NICE, SIGN).
Identify and resolve medication-related problems including adverse effects, interactions, adherence issues, and inappropriate prescribing.
Develop and implement personalised medication management plans in collaboration with the multidisciplinary team.
Provide expert advice and support to GPs, nurses, and other healthcare professionals on complex pharmacological therapies.
Educate and counsel patients to enhance understanding of their conditions and medications, promoting self-management and adherence.
Monitor clinical outcomes and liaise with secondary care specialists and community services as required.
Document all clinical interventions accurately in patients medical records.
Participate in clinical audits, quality improvement initiatives, and service development projects related to chronic disease management.
Stay up to date with the latest developments in pharmacotherapy and primary care medicine.
