Job Description

- See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma).
- Undertake structured medication reviews with patients and produce recommendations for agreement on prescribing changes and monitoring.
- Provide patient facing clinics for those with questions, queries and concerns about their medicines in practices.
- Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
- Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists.
- Signposting to other services and/or healthcare professionals where appropriate, while working within scope of practice and limits of competency.
- To manage repeat prescription requests, resolving queries where possible within scope of practice.
- To maintain full and complete records of all patient contacts using appropriate clinical templates and coding .
- Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate.
- The post holder will be supported to develop their role to become a non medical prescriber.
- Work with practices to ensure full compliance with Care Quality Commission standards for safe and effective care.
- Clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
- Completing the Clinical Pharmacist in Primary Care Pathway and become a prescriber to support the Practice Team.
- Be responsible for the pro-active care management of patients with complex polypharmacy.
- Provide specialist expertise in the use of medicines whilst helping to address public health and social care needs of patients and tackle health inequalities;.

- Membership of the Royal Pharmaceutical Society A member of or working towards Faculty membership of the Royal Pharmaceutical Society Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience Independent prescriber or working towards/intent of gaining independent prescribing qualification.
- Minimum of 2 years post qualification experience.
- Experience working in a GP practice.
- Experience of working in primary care.

Job summary
We are looking for a Clinical Pharmacist, to work across Cordwallis Rd and Woodlands Park GP Surgeries. We are looking for a Clinical Pharmacist who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Senior Clinician and Line Manager who will develop, manage and mentor them.
Required 30 hrs per week.
