      Emergency Department Pharmacist for MercyOne West Des Moines Medical Center.

      Emergency Department Pharmacist - West Des Moines Medical Center


      Clinical pharmacists are responsible for optimizing pharmacotherapy regimens, ensuring safe patient care practices, and advocating for evidence-based medicine within interdisciplinary healthcare teams. As decentralized team members they will conduct daily medication therapy reviews, consults, and other clinical services for patients within designated area(s) of responsibility or targeted populations served such as med-surg, telemetry, oncology, pediatrics, critical care, or emergency medicine. They will follow established policies, procedures, protocols, standards, and regulatory requirements in executing job responsibilities as well as contribute to the pharmacy’s mission to improve health outcomes in our community.

      Required Education and Experience

      1. Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy.
      2. Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience required.
      3. Successful completion of PGY-2 residency or 3 years of experience in the relevant clinical practice area preferred.
      Required Licensure and Certifications 1. Licensed, or eligible for licensure, pharmacist in Iowa. Must be licensed within 3 months of hire.
      2. BLS certification within 3 months of hire.
      3. ACLS and/or PALS certification within 3 months of hire based on practice area(s).
      4. BPS Board Certification (BCPS or specialty board certification in the relevant clinical practice area) within 2 years of hire or 10 years of experience in the relevant clinical practice area.
      5. Other certifications if requested by direct supervisors/managers.
      6. Proof of completion of Mandatory Reporter abuse training for child and dependent adult abuse within three (3) months of hire.
      Required Minimum Knowledge, Skills, Abilities and Training Knowledge: Achieves and maintains an adequate knowledge base including pharmacology, therapeutics, pharmacokinetics, therapeutic drug monitoring, and literature evaluation to provide clinical services for all patient populations and age groups served. Provides clinical services as required by policy and procedure.
      Skills:
      1. Effective communication with patients and other healthcare professionals.
      2. A high level of analytic ability to research and evaluate drug information, interpret patient specific information, and resolve medication related problems.
      3. Displays enthusiasm for teaching and is proficient in all preceptor roles (instructing, modeling, coaching, or facilitating).
      4. Proficient in use of computers, software for pharmacy and healthcare applications, specific pharmacy practice technology and equipment.
      5. Efficiently and adequately document clinical activities and interventions such as consults, patient education, and other patient specific information as defined by policy and procedure.
      6. Ability to utilize problem solving skills to promote a positive work environment and interpersonal relationships, while maintaining professionalism and confidentiality.
