- Performs interactive, patient-centered, person-to-person, telephonic comprehensive medication reviews with members.
- Uses motivational interviewing skills to improve a member’s adherence to medication in real-time phone calls
- Evaluates medication profiles to determine if changes are warranted to optimize the member’s drug therapy and minimize the potential for adverse drug reactions.
- Recommends alternative medications, alternative dosing, or other modifications (including discontinuation of medications) as appropriate.
- Reviews therapy plans with physicians as needed.
- Works in a multidisciplinary team environment to develop and enhance MTM program and the software used to manage the program.
Review member’s medications over the phone and make phone calls to provider’s offices in order to recommend possible changes and or discontinuation of drug therapies due to duplication, interactions or not medically necessary therapies and more.
The pharmacist will be expected to manage all medication reviews over the phone and will be expected to be on the phone for a full 8-hour shift.

Position Purpose: The Medication Therapy Management Clinical Pharmacist provides specialized review of patient profiles and makes recommendations for modifications of drug therapy.
Education/Experience: Bachelor’s or Doctorate of Pharmacy degree. Minimum one year pharmacy residency training completed (or equivalent training/experience) recommended. 1-2 years of experience working within a pharmacy or related health care environment required.
License/Certification: Current, valid & unrestricted state Pharmacy license required and must maintain. Based on work assignment, may be required to obtain additional Pharmacy licensure in multiple states within six months of employment.Working shift - 9am - 6pm Monday -Friday (In your own time zone)
