- Management of patients with chronic diseases to proactively manage patients with complex polypharmacy.
- Clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
- Provide specialist expertise in the use of medicines whilst helping to address public health and social care needs and tackling inequalities across the Primary Care Network geography.
- Develop relationships and work closely with other pharmacy professionals across the wider health and social care economy.

- General Practice/ Primary Care Experience Experience of Case Management Independent Prescribing
- Successful completion of a Great Britain or Northern Ireland accredited MPharm degree
- Passed a registration assessment conducted by the GPhC
- Currently registered with GPhC
- Independent Prescribing Qualification
- Clinical skills and diagnostics qualification
- Clinical Leadership skills
- Management of patients with long term conditions

Job summary
An exciting opportunity has arisen for a Clinical Pharmacist to develop a pioneering role within primary care. West Lancashire GP Federation is looking for Clinical Pharmacist to join the Northern Parishes PCN.
The Clinical Pharmacist is very much a patient-facing role and there will be plenty of opportunity to utilise your clinical skills to improve the health of the local community. This will include managing long-term conditions, supporting hospital discharge prescribing arrangements, providing specific advice for those on multiple medications and supporting the GP practices involved to develop the clinical pharmacist role. You will act as a specialist resource to answer medicines-related queries, to signpost to other services and to liaise with other healthcare professionals as required.

Main duties of the job
We are looking for candidates who are enthusiastic and adaptable to change, with excellent clinical skills and a good level of competence in use of IT. Candidates should exhibit good organisational skills and ability to communicate effectively with a wide range of healthcare professionals and other stakeholders including patients, families and carers. The successful candidate will work independently at network and practice level, but also as part of a supportive practice team, to deliver safe and effective use of medicines by applying their clinical and pharmaceutical skills to a variety of tasks within the medicines optimisation agenda.
About us
Employment will be with Out of Hours West Lancashire CIC Limited (OWLS). We host roles on behalf of the three West Lancashire Primary Care Networks (PCNs) and their member practices.
OWLS is a small GP owned and led not-for-profit primary care organisation run by GPs and health professionals. As a not-for-profit organisation all the money we generate through service contracts is reinvested in providing patient care.
We were founded in the 90s by a small group of GPs to provide high quality out of hours services. In 2017, we became the West Lancashire GP Federation. The Federation supports and provides services directly and with partners, for West Lancashire GP practices and provides a vehicle to bid for and provide primary care services.
Northern Parishes Primary Care Network have 5 member practices and covers a population of 29,000 patients in Burscough, Parbold and Tarleton as well as their surrounding areas. The GP practices that you will support in this role are: Burscough Family Practice, Lathom House Surgery, Parbold Surgery, Stanley Court Surgery and Tarleton Group Practice.
