Job summary
Chorley Together Primary Care Network are seeking an experienced, forward thinking and enthusiastic Clincial Pharmacist to join the team at Dr Dawoud's Surgery on a part time basis. If you're an independent prescriber and looking for a new challenge, then this could be the role for you.

Main duties of the job
The postholder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide structured medication reviews, manage long term conditions, medication reviews for care home patients, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations processes.
The post holder will also provide leadership on quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework.
About us
Formed in May 22, Chorley Together PCN consists of six GP practices across the Chorley and Leyland locality and serves a population of approximately 38000.
Dr Dawouds Surgery is a semi rural practice located in Clayton- Le-Woods, Chorley with good motorway links and on site parking. You will work closely with their GPs (2 Partners and 1 Salaried), Social Prescribing Team, Mental Health Practitioner, GP Assistant, Care Co-ordinator and practice nursing and admin teams as part of a multi-disciplinary team to deliver a holistic and patient centred approach to care.

Key Duties and Responsibilities:
Practice as an independent prescriber working within scope of practice
Carry out face-to-face and telephone consultations with patients for Structured Medication Reviews
To provide medication review services with patients in the practice and during domiciliary visits to the local nursing home
To provide advice and support for patients as required
Encourage cost-effective prescribing within the practice and network
Review all safety alerts ensuring the required actions are taken at practice level
To reconcile medicines following patients discharge from hospital or clinic appointments
Implement and embed a robust repeat prescribing system
To provide subject matter expertise on medication monitoring, implementing and embedding a system
Carry out targeted medication reviews for high-risk patients
To reauthorise medication for repeat prescribing within scope of practice
Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required
Review and implement safe prescribing systems for vulnerable patients, improving compliance
Support clinicians with the management of patients suffering from drug and alcohol dependencies
Actively signpost patients to the correct healthcare professional
Assist with QOF targets
In addition to the key responsibilities, the clinical pharmacist may be requested to:
Partake in audit as directed
Provide education on medicines for the clinical team
Review the latest guidance ensuring the practice conforms to NICE, CQC etc.
Act as the practice point of contact for all medicines related matters, establishing positive working relationships
Contribute information to help PCN, commissioners and local partners identify unmet needs within the community and gaps in community provision.
Contribute to PCN work by meeting the requirements of the PCN DES
