- The post holder will work within their clinical competencies to provided specialist pharmaceutical support to the Practices clinical team to improve care and support safe and effective prescribing
- The role will involve providing patient facing services either via face to face or telephone consultations and will undertake chronic disease management, repeat prescription management and structured medication reviews.
- The post holder will need to be proactively involved in the quality improvement and audit as well as managing some aspects of the Quality and Outcomes Framework (QOF).
- Clinical medication reviews for patients with single of multiple long-term conditions where medicines optimisation is required
- To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and to reduce risk of readmission
- To be involved in multidisciplinary clinical meetings at the Practice. where appropriate
- To manage repeat prescription requests, resolving queries where possible within scope of practice. To ensure patients have appropriate monitoring tests in place when required.
- Implement changes to medicines in line MHRA alerts, product withdrawal or shortage and other local or national guidance.
- To maintain full and complete records of all patient contacts using appropriate clinical templates and coding
- To undertake a proactive role in audit and quality improvement implementing recommendations where appropriate
- Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate
- Participate in review and setting of policies relating to medicines management and prescribing and help ensure the Practice prescribes in accordance with local guidelines and formulary
- To identify areas of clinical risk at Practice level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of Practice resources.
- To maintain own clinical and professional competence and be responsible for own continuous professional development.

About us
Pendeen Surgery is a busy market town GP Practice with just under 9000 patients. At Pendeen Surgery we have an excellent group of clinical and admin individuals who enjoy working alongside each other. Primary Care has seen many challenges in recent years and our team is always supportive and positive towards one another.
We are training Practice with three GP trainers (Dr Das, Dr Boughen and Dr Thurgood). We use EMIS Web and Docman clinical systems, along with Accurx and Lexacom ECHO. We have consistently been rated as Good by CQC and are very high QOF achievers.
We are part of the South and West Primary Care Network (Herefordshire). Currently our Extended Access cover is provided by Taurus Healthcare with appointments in the evenings and at the weekends. From 1st April 2025 the Practice will be covering the Extended Access at the Surgery rather than at PCN locations. We also have Herefordshire Remote Health (HRH) virtual appointments during the day which is also run by Taurus Healthcare.
The surgery is situated just 5 minutes out of town adjacent to Dean Hill Park and a stone's throw from the Community Hospital.
