What to Expect (Job Responsibilities):
- Lead assigned patient service lines and therapeutic programs, providing direct patient care
- Conduct in-clinic visits, follow-up monitoring, patient education, and medication reconciliation
- Coordinate access and affordability of specialty medications for patients
- Participate in pharmacy operations and medication dispensing within the medical center
- Facilitate experiential education and practice advancement for pharmacy students and residents
What is Required (Qualifications):
- Graduate of an ACPE accredited School of Pharmacy
- Completion of ASHP accredited PGY-1 Pharmacy Residency
- Significant knowledge and expertise in advanced pharmacy practice and specialty medication therapy management
- Strong written and verbal communication skills
- Eligible for licensure as a pharmacist in the state of Illinois within 60 days of employment
How to Stand Out (Preferred Qualifications):
- Doctor of Pharmacy or another advanced degree
- PGY-2 Pharmacy Residency highly preferred
- Clinical pharmacy experience (2-4 years) in a hospital setting
- Pediatric experience
