What to Expect (Job Responsibilities):
- Lead assigned patient service lines, clinical areas, and therapeutic programs
- Monitor patient follow-ups, provide education, and manage medication reconciliation
- Coordinate care for specialty medications to ensure access and affordability
- Deliver direct patient care and participate in pharmacy operations
- Facilitate experiential education and precepting for pharmacy students and residents
What is Required (Qualifications):
- Graduate of an ACPE accredited School of Pharmacy
- Completion of ASHP accredited PGY-1 Pharmacy Residency
- Significant knowledge and expertise in advanced pharmacy practice and specialty medication therapy management
- Strong written and verbal communication skills
- Eligible for licensure as a pharmacist in the state of Illinois or eligible to complete the licensure process within 60 days of employment
How to Stand Out (Preferred Qualifications):
- Doctor of Pharmacy or another advanced degree
- Completion of PGY-2 Pharmacy Residency
- 2-4 years of clinical pharmacy experience in a hospital setting
- Pediatric experience
