Job summary
We are looking to welcome an experienced Clinical Pharmacist to join our dynamic and supportive team at Clifton lane Medical Centre .This vacancy is up to 30 hours per week, salary depending on experience.
Main duties of the job
Were looking for enthusiastic and forward-thinking pharmacist to complement the team. The successful candidate will hold clinics for patients requiring medication reviews, manage prescription queries and be involved in long term condition management. We welcome applicants with or without GP experience. If you have the right skills and qualities, we can provide on-going support to develop professionally.
About us
Clifton Medical Centre's list size is 5500. We have three GP Partners, Salaried GP, Specialist nurse, practice nurse, Clinical Pharmacists along with other admin team. Clifton Medical Centre has a branch site Victoria Health Centre based in Smethwick.

Undertake structured medication reviews with multi morbidity and polypharmacy and implement changes whilst ordering relevant monitoring.
Collaborate with the wider healthcare team to improve safety of medicines ordering and administration.
Provide support for the pharmacy technician for management of medicines following hospital discharge and clinic visits.
