- Evidence of extensive Surgical nursing / AHP experience at Band 5 level or above

Job summary
This is an exciting and innovative opportunity for an individual who has a keen interest in digital health and technology. You will be responsible for supporting flow and capacity across the surgical division by enabling suitable patients to either have an earlier discharge or to prevent an inpatient admission, by providing acute hospital level care within their own homes.
This post requires on-site working at Gloucestershire Royal Hospital, and occasionally working across site at Cheltenham General Hospital. This is not a remote working role
Main duties of the job
Day to day management of cohorts of patients who are being cared for remotely; using digital technology to monitor and provide elements of their care. Working within local and national guidelines to provide assessment, treatment plans, escalations and evaluations.
Part of the overarching Trust Virtual Ward / Hospital@Home service, working within a multi professional team, working closely with acute and community practitioners.
